Produce Evidence & IdeasSouthwest Houston Community Health Survey Initiative: With support from the Memorial Hermann Community Benefit Corporation, and in collaboration with five community partners, Texas Health Institute conducted a first-of-its-kind electronic, multilingual, in-person field survey in Southwest Houston to identify the lived experiences and perspectives of diverse communities on the social determinants of health. Findings from this initiative provide timely, actionable community data to inform and drive collaborative dialogue and action to improve the health of Southwest Houstonians. Health Opportunity & Equity (HOPE) Initiative: Led by the National Collaborative for Heath Equity and Texas Health Institute in collaboration with Virginia Commonwealth University’s Center on Society and Health and funding from Robert Wood Johnson Foundation, HOPE provides a new approach with actionable data and tools for measuring national and state progress toward equity. HOPE examines 28 indicators of well-being, including health outcomes, socioeconomic factors, social environment, physical environment, and access to health care, with national and state benchmarks and estimates by race, ethnicity, income, and education. Translate & Share InsightsAmerica’s Health Rankings Health of Those Who Have Served Reports: Since 2016, Texas Health Institute in collaboration with Arundel Metrics, Inc. and funding from the United Health Foundation has developed four veterans’ health reports for America’s Health Rankings’. With the objective of informing policies to improve veterans’ health, these reports provide actionable data and insights that compare the broader health of men and women who have served in the U.S. Armed Forces to civilians, overall and by age, race/ethnicity, and socioeconomic factors. The Nueces County government and hospital district have launched an innovative effort to develop a health equity report and data analytics platform. The combined offering will provide actionable insights to inform policy-making and programs that improve community and health conditions for Nueces County residents amid the COVID-19 pandemic and into the future. The project will be conducted by THI, leading the assessment of COVID-19 and community health needs and inequities, review of existing policies, and key stakeholder engagement. Professional services firm Accenture will provide technology services, data science and analytics capabilities in collaboration with THI for the research, reporting and development of a new actionable data dashboard for Nueces County. Monitoring the Affordable Care Act from an equity lens: Over the past few years, our team has produced over a dozen reports to share insights on the opportunities, implications, and impact of the Affordable Care Act for addressing racial/ethnic health disparities and achieving equity. Funded by W.K.Kellogg Foundation, Connecticut Health Foundation, The California Endowment, Blue Shield of California Foundation, and others, these reports cover wide-ranging health reform topics such as the Health Insurance Marketplace, Enrolling Diverse Populations in Coverage, & Safety-Net Hospital Systems Transformations. Advance Collaborative ActionIn the Wake of the Affordable Care Act - Understanding Community Barriers and Facilitators to Health Care Access: With the purpose of advancing collaborative action in South Sacramento, we utilized a community-based participatory approach for measuring, monitoring, and addressing health care access concerns from a social determinants perspective. This involved developing and administering a community-based survey in multiple languages, training a cadre of community surveyors for ongoing evaluation, and engaging community leaders in dialogue to promote advocacy and action.
